Our Substance Use Disorder (SUD) Program supports individuals in treatment by helping them prepare for stable housing and essential services upon discharge. Through early planning, coordination with treatment providers, and hands‑on case management, we help reduce gaps in care and support long‑term recovery.
The Moving Home Minnesota program is committed to supporting individuals in SUD treatment centers and is designed to bridge the transition from treatment to community living. By focusing on housing stability, service coordination, and individualized planning, our team works alongside clients and treatment partners to create realistic, supportive discharge plans that promote safety, dignity, and recovery.
Exodus PCS creates and follows a structured, proactive plan that begins during treatment and continues through discharge planning. Services include eligibility verification, intake coordination, housing planning, and service authorization. This approach allows our team to anticipate needs early and act quickly as discharge dates approach.
In-person and remote options
Housing is a core goal of the MHM SUD Program. Our team works with clients, treatment centers, and community partners to identify qualified housing options, explore transitional supports, and develop backup plans when needed. This focus helps clients exit treatment with safer, more stable living arrangements.

I had the blessing of having Joyce O. as my relocation worker. I am blown away by how resourceful she was! Not only is she kind, prompt, and patient; she also OBVIOUSLY really cares about her clients! Thanks to Joyce, I was able to find the perfect place to call home! Navigating a search for housing and programs seemed liked an IMPOSSIBLE venture but thanks to Joyce, I learned about what I am eligible for. Joyce is a dedicated and compassionate worker that deserves MORE than just 5 stars!! I hope that she gets all of the praise and accolades that she deserves!!

My dad’s case manager Alexis partnered with us the WHOLE way. She waited on hold with the county and hunted down answers if she didn’t know them. She was patient and understanding and she actively PARTNERED with us rather than doing it all- allowing my dad the ability to be a part of his own team rather than having things done for him. I honestly don’t know how I would have managed this life-altering and confusing transition with my dad if we hadn’t had Alexis from Exodus. Other staff at Exodus were also super helpful in navigating several stressful situations. And you know what? Just having someone validate/acknowledge my own struggles and challenges associated with supporting my dad in his own independence, really eased the burden

Scott from Exodus PCS was invaluable in finding a new home for my brother. The long-term care center my brother lived in closed and we were on a time crunch to find him a new home. Scott helped find a group home that specializes in traumatic brain injuries which is essential for my brother's care. Although the process was intimidating. I was assisted throughout the whole transition. Scott is knowledgeable, personable and has great compassion for the clients and community that Exodus PCS serves. I am forever grateful for the support and guidance I received from Scott and Exodus PCS!

Connie has been amazing throughout this journey of trying to get my loved one out of the nursing home (TCU). Without her assistance I don’t think we could have done it without her. Connie is very knowledgeable about the community resources and willing to go the extra mile for my family member to assist him like it was her family member. Connie is very compassionate and gave me the support I needed and guidance to get my love one back into the community. Without Exodus and Connie’s assistance I don’t think it would have happened. We are so greatful.
