WHAT IS RELOCATION SERVICE COORDINATION?
Relocation Service Coordination (RSC) is a Medical Assistance (MA) funded program that assists person’s living in Nursing Facilities find housing in the community. It is a form of Targeted Case Management (TCM) that identifies a person’s medical and social needs and works in collaboration with the individual and their team to safely transition into their new home.
Exodus coordinates with various counties and managed care organizations to help individuals gain access to services and supports. We create a Person-Centered transition plan to deliver choice and ensure supports are in place at your new home!
ELIGIBLE RECIPIENTS
Contact us today about your eligibility. A recipient is eligible to receive RSC-TCM services when all of the following criteria are met:
Eligible for MA
Resides in an institution that qualifies for services at the time of service delivery
Chooses to move into the community
Chooses to receive services
TCM benefit is not exhausted
