Exodus PCS works to create a person centered relocation plan designed to meet the needs of each client and promote a successful transition into the community. We help identify and access various resources and supports in the community to ensure a smooth transition. We pride ourselves on listening to the needs of our clients and strive to give them every opportunity available.

We value your choices in the community and it's our mission to connect you with the services you need to maintain your health and good safety. We will discuss the multiple housing options available to you, explain how moving will affect your benefits and income, and support those enrolled through a smooth transition into the community.
